Precision Perioperative Nursing under the Enhanced Recovery After Surgery Framework: A Narrative Review of Evidence, Core Interventions and Future Directions

Abstract

Enhanced Recovery After Surgery (ERAS) has reshaped perioperative care by integrating evidence-based, multidisciplinary interventions to reduce surgical stress, accelerate functional recovery, shorten hospital stay, and improve patient-centered outcomes. Nursing is central to ERAS implementation because many key components, including preoperative education, nutritional screening, pain assessment, early oral intake, early mobilization, tube and drain management, discharge preparation, and post-discharge follow-up, are nurse-sensitive processes. In recent years, ERAS nursing has evolved from standardized pathway implementation toward precision perioperative nursing, in which interventions are tailored according to surgical type, patient frailty, nutritional status, comorbidities, psychological state, functional capacity, and recovery trajectory. Current evidence suggests that ERAS-based nursing programs can reduce postoperative complications, facilitate gastrointestinal recovery, improve early mobility, and enhance patient satisfaction across colorectal, thoracic, orthopedic, gynecologic, hepatobiliary, pancreatic, obstetric, and transplant surgery. However, challenges remain, including variable protocol adherence, insufficient risk stratification, inconsistent outcome measures, limited digital integration, and the need for stronger long-term follow-up. This review summarizes the development of ERAS nursing, core precision interventions, evidence across surgical specialties, implementation challenges, and future research priorities. It also emphasizes that the future value of ERAS nursing depends not only on completing protocol items, but also on identifying patient-specific barriers, supporting shared recovery goals, and extending perioperative care beyond discharge.
